As an AOA Member under age 65 currently residing in the US.*, you and your spouse can now apply for up to $1,000,000 (in multiples of $10,000) in Opti-Life coverage as a convenient “add-on” to any coverage you may have through work or on your own. Many families rely on two incomes these days. That’s why this benefit offers coverage for you and your lawful spouse (under age 65) and unmarried children between 15 days and 23 years (25 if full time student). A dependent who is also a member is eligible for either member or dependent coverage, but not both. If both the member and spouse are covered as members, neither may insure the other as a spouse. If both parents are insured as members, only one may request child coverage. In addition, the AOA Opti-Life Insurance Plan has features that may make this Group Term Life coverage stand out from other programs:

Benefits Double If You Fall Victim To A Fatal Accident
In the event of a covered fatal accident, your loved ones will collect additional Accidental Death Benefit that doubles your life benefit amount not to exceed $500,000. For instance: If you’re insured for $100,000 in life coverage, your family would get $200,000 benefits if you fell victim to a fatal car accident. This accident coverage is automatically included in Opti-Life for you at no additional cost.
Request Up to 50% Of Your Benefits Early If Terminally Ill
Terminal illnesses like cancer can have a devastating impact on your family finances. That’s why the AOA negotiated the accelerated death benefit that allows you to request up to 50% of your life benefits before your death if you’re diagnosed as terminally ill with less than 12 months to live. The request must be made at least 12 months prior to your scheduled coverage termination age and the amount of insurance benefits payable after your death will be reduced by this payment. This benefit can be paid only once. Premium contributions do not reduce. To qualify, you must provide the insurance company with proof of terminal illness and anticipated life expectancy (12 months or less), as well as any other necessary medical information requested. Find your age bracket in the Monthly Rate Chart. Decide the amount of desired coverage. Locate the premium amount listed under Non-Smoker or Smoker. Benefit amounts available: $10,000-$1,000,000.
For example, a 39-year-old non-smoker, who selected $50,000 in coverage, would have a total monthly premium of $3.15.

+The current 20% premium credit is not guaranteed and subject to change, however, the AOA Group Term Life Insurance has returned premium credits for several years.
There will be a $3.00 administration fee per billing for direct bill payment, $2.00 administration fee per billing for credit card payments, or a $1.00 administration fee per billing for automatic bank deductions. The administrative fee is waived if paying annually.
Premiums will not change. Rates shown are monthly for Members and spouses. For your convenience, you will be billed quarterly. Premium shown is based on age and whether you are a non-smoker at the time of enrollment and is adjusted on the first premium due date following entrance into a new age bracket. Rates and or benefits may be changed on a class basis. Coverage will reduce following the date you reach age 65. The total amount of life insurance an individual may have in force under all group insurance benefits underwritten by New York Life Insurance Company may not exceed $1,000,000.

The premium contributions shown reflect the current rate and benefit structure. Premiums increase on the premium due date coinciding with or next following the date that a member enters a new age bracket. Premium contributions may be changed by New York Life Insurance Company on any premium due date and any date on which benefits are changed. However; your rates may change only if they are changed for all others in the same class of insureds under this group policy. For example, a class of insureds is a group of people all with the same issue age, waiting period, and benefit option amounts are not guaranteed and are subject to change by agreement between New York Life Insurance Company and the AOA.

Why should I consider adding this AOA benefit if I already have coverage through work?
There are two important reasons many doctors of optometry add the AOA Opti-Life to their family’s financial safety net. First, even if you already have coverage through work, it may not be enough to meet the full life insurance needs of your family. And secondly, if you decide to change jobs or open your own practice, those benefits through a previous employer will likely end. But this coverage isn’t tied to any job or any company. So you can rest assured your coverage can stay in force. No matter what changes occur in your career, as long as you pay your premiums when due and remain an AOA member, the policy remains in effect.
What happens if I fall victim to a sudden accident? Will the AOA Opti-Life still deliver benefits for my loved ones?
Absolutely. In the event of a covered fatal accident, your loved ones will collect additional Accidental Death Benefit that double your life benefit amount. For instance: If you’re insured for $100,000 in life coverage, your family would get $200,000 benefits if you fell victim to a fatal car accident. This accidental death coverage is automatically included in the group term life for you and your spouse at no additional cost.

Effective Date of Coverage
Insurance will take effect on the first of the month on or following the date your application is approved by New York Life Insurance Company, provided the initial contribution is paid within 31 days after you are billed and any person to be insured is performing normal activities of a person in good health of like age (NC Residents: a person of like age) on the day of approval. If any dependent proposed for coverage is not performing his/her normal activities as required, coverage will not take effect until he/she is performing such activities provided such date is within three months after the date insurance would have been effective and the person is still eligible.
When Coverage Ends
(a) You are no longer an employee of AOA, (b) the premiums are not paid when due, (c) the group policy is terminated or modified by the policyholder to end insurance for the group of insureds to which the member belongs, and (d) if the insured requests to terminate insurance. In addition, dependent coverage will terminate when the dependent spouse or child ceases to be an eligible dependent.
Definitions:
Total Disability means an incapacity from an injury or sickness which completely and continuously prevents an insured member from doing the material and substantial duties of any occupation for which he or she is reasonably qualified by education, training or experience; results in total and permanent loss of sight of both eyes; or results in severance, above the wrist or ankle of both hands, both feet or one hand and one foot.
Beneficiary Designation
You can name any beneficiary you wish for member coverage. You may change this beneficiary at any time, by written request. Your beneficiary is the person(s) last designated by you in writing, and recorded by or on behalf of New York Life Insurance Company. You are the automatic beneficiary for dependent insurance, as described in the Certificate of Insurance. If you wish to name another beneficiary for dependent spouse or child insurance, contact the administrator at 1-866-331-0180 for the applicable form.
Exclusions & Limitations
In the case of death due to suicide (whether sane or insane), an attempt at suicide or intentionally injuring oneself within 12 months of coverage, the benefit will be limited to the return of premiums paid.
The accidental death and dismemberment benefit does not cover loss resulting from: intentionally self-inflicted injury, suicide or attempted suicide, while sane; Occurs during, is due to or related to the insured member or spouse’s duty in the military, naval or air services of any country; participation in or incarceration resulting from any of the following, other than as a victim, commission of a felony, illegal occupation or activity, an insurrection or terrorist activity; injury sustained while riding in or on, fall from or decent from any aircraft while in flight unless the insured is traveling solely as a passenger; sickness or disease or medical or surgical treatment of sickness or disease, except infections which occur as the result of an accidental bodily injury; or accidental ingestion of contaminated material, any medical, dental or surgical treatment unrelated to the accident, engagement in any of the following in a role other than as a victim: war, an act of war, or an armed conflict which involves the armed forces of one or more countries, or voluntary entry into the body of an intoxicant, excitant, hallucinogen or any narcotic or other drug or any gas, poison or poisonous substance.
We will not pay more than the Principal Sum for all losses due to the same accident, but will pay the maximum benefit amount available as a result of any one accident. The Accidental Death and Dismemberment Benefit Principal Sum will reduce in accordance with the Reduction in Amount of Life Insurance provision. Injury means bodily injury resulting directly and independently of all other causes from an accident which occurs while you are covered under the policy.

Important Information from New York Life Insurance Company
New York Life Insurance Company reserves the right to request medical information to determine applicant’s medical eligibility for coverage. Based on the age of the person proposed for insurance and the amount of coverage requested, a physical examination, EKG, blood test or other information may be required. Not all applicants will have to supply additional information. However, if it is required, New York Life will arrange for an independent professional paramedic to contact you to perform these simple tests at your convenience. The exam and blood test are free-of-charge.
If we can provide the coverage you requested, we will inform you as to when such coverage will be effective. Under no circumstances will coverage be effective prior to this date. Payment of a premium contribution with your application does not mean that there is any insurance in force before the effective date as determined by New York Life.
IMPORTANT NOTICE:
How New York Life Obtains Information and Underwrites Your Request For Group Term Life Insurance
In this notice, references to “you” and “your” include any person proposed for insurance. Information regarding insurability will be treated as confidential. In considering whether the person(s) in your request for insurance qualify for insurance , we will rely on the medical information you provide, and on the information you AUTHORIZE us to obtain from your physician, other medical practitioners and facilities, other insurance companies to which you have applied for insurance and MIB, LLC (“MIB”). MIB is a not-for-profit organization of insurance companies, which operates an information exchange on behalf of its members. If you apply for life or health insurance coverage, a claim for benefits is submitted to an MIB member company, medical or non- medical information may be given to MIB, and such information may then be furnished by MIB, upon request, to a member company.
Your AUTHORIZATION may be used for a period of 24 months from the date you signed the application for insurance, unless sooner revoked. The AUTHORIZATION may be revoked at any time by notifying New York Life in writing at the address provided. Your revocation will not be effective to the extent New York Life or any other person already has disclosed or collected information or taken other action in reliance on it, or to the extent that New York Life has a legal right to contest a claim under an insurance certificate or the certificate itself. The information New York Life obtains through your AUTHORIZATION may become subject to further disclosure. For example, New York Life may be required to provide it to insurance, regulatory or other government agencies. In this case, the information may no longer be protected by the rules governing your AUTHORIZATION.
MIB and other insurance companies may also furnish New York Life, its subsidiaries or the Plan Administrator with non-medical information (such as driving records, past convictions, hazardous sport or aviation activity, use of alcohol or drugs, and other applications for insurance). The information provided may include information that may predate the time frame stated on the medical questions section, if any, on this application. This information may be used during the underwriting and claims processes, where permitted by law.
New York Life may release this information to the Plan Administrator, other insurance companies to which you may apply for life and health insurance, or to which a claim for benefits may be submitted and to others whom you authorize in writing, however, this will not be done in connection with test results concerning Acquired Immune Deficiency Syndrome (AIDS) or Human Immunodeficiency Virus (HIV). We may also make a brief report of your protected health information to MIB, but we will not disclose our underwriting decision.
New York Life will not disclose such information to anyone except those you authorize or where required or permitted by law. Information in our files may be seen by New York Life and Plan Administrator employees, but only on a “need to know” basis in considering your request. Upon receipt of all requested information, we will make a determination as to whether your request for insurance can be approved.
If we cannot provide the coverage you requested, we will tell you why. If you feel our information is inaccurate, you will be given a chance to correct or complete the information in our files. Upon written request to New York Life or MIB, you will be provided with non-medical information. Generally, medical information will be given either directly to the proposed insured or to a medical professional designated by the proposed insured. Your request is handled in accordance with the Federal Fair Credit Reporting Act procedures. If you question the accuracy of the information provided by MIB, you may contact MIB and seek a correction. When program experience has been good New York Life Insurance Company will periodically return excess premiums in the form of dividends to the AOA as the policyholder. The AOA will retain these dividends in a Dividend on Deposit Account with New York Life. The AOA will in turn periodically make these dividends available through premium credits to existing insureds in the programs which generated the dividends. Dividends are not guaranteed. Sufficient reserves will be retained by the AOA in the Dividend on Deposit Account to cover administrative and marketing costs generated by the New York Life Insurance Programs sponsored by the AOA. These programs are administered together to take advantage of the savings resulting from this integrated approach.
Administrative expenses incurred by the AOA to provide the valuable membership benefits resulting from these sponsored insurance programs are reimbursed from available program dividends. New York Life may also, out of premium, pay a reasonable fee to the AOA for making the AOA assets available to it to promote these programs to the membership. These assets include the AOA Intellectual Property Rights and mailing lists of eligible members.
